    Job Description:



    We are looking for experienced Home Health Physical Therapists (PTs) who can work independently in delivering quality care. Responsibilities include performing initial and ongoing assessments, developing individualized plans of care, providing skilled interventions for strengthening, endurance, pain management, and fall prevention.
