    • HOSPICE Serenity Hospice Care is looking to hire 'Registered Nurse (RN) Case Managers to join our team of professionals.
    Available territories include Bucks, Delaware & Philadelphia Counties and the surround area.

    The RN Hospice Case Manager will be responsible for assisting the Team with Case Management/Visits to aid in coverage and call outs.

    Job duties can include, but are not limited to:

    Completing an initial assessment of patient and family to determine hospice care needsRegularly re-evaluates the patients nursing needsUse assessment data to determine care plan needsCollaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinatedCounsels the patient and family in meeting end of life needsInitiates appropriate preventative nursing proceduresCommunicates with the physician regarding patient's needs and reports any changes in the patient's conditionSupervises the Hospice Aides and LPN'sThe proper candidate must have a current New Jersey RN License in good standing, must be familiar with and willing to travel the area and have hospice experience.

    Must also have reliable transportation and insurance. Monday through Friday 8:30am to 4;30pm. Care for Hospice patients in their homes and facilities.
