    ESSENTIAL FUNCTION


    This position exists to ensure that the health care needs of each resident served by Covenant Ability Network of Illinois are met in a thorough and timely manner within the parameters set forth in the agency's licensure.


    QUALIFICATIONS

    To be qualified for this position an individual must meet the following standards and be able to perform the essential functions of the job.

    Must be legally eligible to work in the United States

    Minimum:
    Registered Nurse (RN)

    Valid nursing license from the state of Illinois. Must be in good standing with all credentialing and regulatory agencies

    Prefer 2 years of related experience working in similar environment and experience working with individuals with intellectual and developmental disabilities and supervisory experience.

    Approved DHS-Nurse Trainer
    Must be proficient in Microsoft Office Suite applications, including Word, Excel and Outlook.
    Must pass pre-employment physical and drug test within 3 days of contingent offer of employment.
    Must pass an Illinois state criminal background check with no disqualifying conditions.
    Must pass TB test within 30 days of employment.
    Must have a valid Illinois driver's license with a good driving record to maintain insurability in accordance with MVR.

    RESPONSIBILITIES

    The maintenance of each individual served health care record in compliance with state and federal regulation and agency policies and procedures.

    The completion of all quarterly (by RN Only) and annual nursing assessments for each individual served ensuring that all documentation is completed.

    The ordering, monitor, assessment, and documentation associated with each individual's medication regimen and ensuring lab orders are followed and documents are shared with MD for signatures.

    This will include SAMA's and PSR's per state guidelines.
    Required to follow all requirements as set out by the rule governing the "Delegation of Administration of Medications" (DHS Administrative Rule 116)
    Ensuring that all MAR data matches POS which includes any updates of medications throughout the medication cycle.

    Attendance at each individual's interdisciplinary team meetings at the ICF and attendance to each individual served at a CILA 's yearly meetings.

    Completion of all yearly medical assessments before each individual's annual review.
    Direct oversight of medical budget set forth by approved budget.

    Monitoring of ordering and stock of all medical supplies in the home in coordination with Medical Coordinator and direct support professionals in homes.

    Working with the medical coordinator relating to the medical needs and accompanying persons served by the agency to medical and health-related appointments as needed.

    Training of all DSP's on the medication cart at the ICF and the CILAs to ensure that they are capable of providing medication administration according to State guidelines.

    Providing ongoing training of all DSP's which includes providing competencies throughout the year as per Nurse Supervisor.

    Facilitate, coordinate, and document Minutes and retain a signature of attendance sheet at in-service training to all direct support professionals and staff on health-related issues which may include state mandated by Basic Health and Safety, Medication Class and CPR/First Aid.

    Develops and monitors the quality assurance of the agency's infection and exposure control program.
    Scheduling of all infection control meetings as evidenced by meeting minutes.
    Promote a clean, sanitary, and pleasant personal appearance for effective role modeling to the individuals served.
    Observe and report needed repairs and equipment in need of service.
    Maintain regular communication with family members and other interdisciplinary team members.
    Quarterly, present information and data to the Behavior Management Committee, Psychotropic Medication Review, Human Rights Committee, other agency meetings and meetings with the Executive

    Director/Administrator.

    Works with the Program Coordinator to gather all medical information during the admission, orientation, discharge and transfer of all individuals receiving services.

    Develops and maintains all individual's plan of care.
    Conduct employee performance reviews at least annually for all direct reports.
    Provide and document coaching, teaching, and corrective discipline for staff under direct supervision.
    Responsible for scheduling and conducting interviews with potential staff supervised under this position.
    The coordination of and participation in quarterly pharmacy reviews with the agency's pharmacist consultant.
    Attendance at agency in-service training and staff meetings as required by Executive Director.
    On-call responsibilities
    Research medical reimbursement issues when sanctioned as appropriate by the Executive Director.

    The provision of medical support to aid employee health screening for employment retention and regulatory compliance in administration of TB vaccine, coordination of flu vaccine clinic and Hepatitis B vaccinations or other health and wellness duties as directed by the Illinois Department of Health.

    Support and assistance to the Executive Director or Program Coordinators when sanctioned as appropriate.
    Expected travel to all CILA homes and ICF.
    Participation in state surveys including implementation and in-service training on corrective action plans.
    Develop, revise and maintain health related policy and procedures in coordination with the Executive Director.
    Performance of other duties applicable to resident and facility needs as assigned by Executive Director

    ENVIRONMENTAL CONDITIONS
    Is subject to flexible hours including split shifts or varied hour including mornings, afternoons, evenings, and weekends.
    Sits, stands, walks, bends, lifts, and moves intermittently during working hours.
    Is subject to frequent problem-solving activities.
    Has regular contact with staff, individuals, family members, and the general public.
    May be subject to hostile and emotionally upset individuals, staff, family members and the general public.
    May be exposed to infectious diseases, odors, dust, etc. throughout the day.
    Work location may change with little or no notice.
    May be exposed to pets.
    May be experience frequent interruptions and varying levels of noise.
    On a temporary basis may work in other positions as needs dictate, including evenings, holidays and weekends.

    PHYSICAL REQUIRMENTS
    Manual dexterity is necessary to teach skills and provide care to the individuals receiving services.
