- Prescriptive Authority: Prescribe medications, treatments, and therapeutic interventions within the scope of practice and in accordance with state regulations and practice guidelines.

- Chronic Disease Management: Develop and implement individualized care plans for patients with chronic conditions, monitoring disease progression, managing symptoms, and coordinating multidisciplinary care to optimize outcomes and quality of life.
- Acute Care Management: Evaluate and manage acute illnesses and injuries, including minor injuries, infections, and exacerbations of chronic conditions, providing timely and appropriate interventions and referrals as needed.
- Collaborative Care: Collaborate with physicians, specialists, nurses, and other healthcare professionals to coordinate comprehensive care and services for patients, ensuring continuity of care, effective communication, and interdisciplinary collaboration.
- Documentation and
- Quality Improvement: Participate in quality improvement initiatives, performance monitoring, and outcome measurement activities to enhance the delivery of primary care services and promote patient safety and satisfaction.
- Master of Science in Nursing (MSN) degree from an accredited nurse practitioner program.
- Current licensure as a Nurse Practitioner (NP) and Registered Nurse (RN) in the state of practice.
- Certification as a Nurse Practitioner in the specialty area of primary care (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner).
- At least 6 months of clinical experience as a Nurse Practitioner in a primary care setting, required.
- Strong clinical assessment, diagnostic reasoning, and treatment planning skills.
- Excellent communication, interpersonal, and patient engagement skills.
- Commitment to providing culturally competent, patient-centered care in a diverse community.
