- To be eligible for an exemption based on Religious Beliefs, the worker must provide a written statement describing the worker's sincerely held religious beliefs and how those beliefs conflict with Colorado's August 21, 2021 Public Health Order requiring certain healthcare workers to be fully vaccinated against COVID-19. The company may request supporting documentation to further evaluate the request for religious accommodation.
- To be eligible for a Qualified Medical Reason exemption, the worker must provide a written statement signed by a physician, physician assistant, advanced practice nurse, or certified nurse midwife licensed in the State of Colorado stating that the COVID-19 vaccination for the employee, direct contractor, or support staff is medically contraindicated as described in the product labeling or authorized by the FDA. This statement should not describe the underlying medical condition or disability but should indicate the probable duration of the inability to receive the vaccine.

About the Position:
Overnight Schedule (7a-7p Friday - Sunday). This position is responsible for crisis evaluations, preauthorization requests, and coordination of admissions to Cedar Springs.
The position consists of answering phone calls, conducting mental health evaluations, evaluating for involuntary commitments, making clinical recommendations for optimal level of care referrals and community adjustment, coordinating with community providers and referral sources, and facilitating internal admissions to Cedar Springs programs.
Needs to perform vitals, breathalyzer tests as well as temperature on each potential patient at time of evaluation. Immediate consideration for overnight schedules.Shift Differentials for Overnight shift: Weekday additional $5.00/hr &Weekend additional $7.00/hr

Requires organizational, verbal, interpersonal and customer service skills, grammar and spelling, the ability to read/comprehend written instructions and follow verbal instructions.
Ability to establish and maintain effective working relationships with employees, managers, regulatory agencies, etc. using strong interpersonal communications skills. Ability to exercise professional independent judgment in planning own work, formulating recommendations and making decisions.Education:
Master's Degree in Social Work or Counseling required. Preferred of at least one year of experience with crisis work and mental health evaluations. Unlicensed and licensed individuals considered. If licensed, Colorado licensure must be in good standing meeting state requirements for enacting involuntary holds (e.

CSW, LPC, MFT) with two years of mental health experience.
