- Monday - Friday, flexibility for weekends if required
- Conducting home visits and phone evaluations to assess potential risks and suitability of placement
- Providing psychosocial education to sponsors on unique needs of minors
- Identifying family strengths and social support systems
- Assisting families in formulating personalized integration plans
- Offering ongoing case management with a focus on screening and appropriate referrals for various needs related to placement, safety, education, health, legal matters, and more
- Completing all necessary reports, case notes, and documentation within specified timeframes
- Collaborating with relevant agencies to ensure minor's safety and well-being
- Providing crisis intervention as needed
- Engaging in community outreach events to enhance available resources for families
- Supporting additional projects related to family reunification program
- Case Manager:
- Bachelor's degree in Human Services, Social Work, or related field
- Minimum of 2 years relevant experience or equivalent combination of education and experience
- Senior Case Manager:
- Master's degree in related field
- 5 years of related experience
- Bilingual proficiency in English and Spanish required
- Proficiency in essential computer systems and software applications
- Valid driver's license and ability to drive (Agency and/or personal vehicle)
- Fluency in both Spanish and English, with cultural competence in dealing with clients from diverse Latino/Hispanic backgrounds
- Strong interpersonal skills
- Ability to maintain composure in high-pressure situations
- Thorough understanding of case management and crisis intervention services
- Exceptional communication skills, both verbal and written
- Ability to act with discretion and professionalism
- Strong organizational and planning abilities
- Primarily sedentary work involving periods of sitting/standing
- Ability to exchange information by communicating with others
- Capacity to lift up to 20 pounds
- Hearing and vision capabilities within the normal range
- Maintaining accuracy and thoroughness in assigned work
