DescriptionJOB SUMMARY:
The Licensed Practical Nurse (LPN) provides compassionate, skilled nursing care to hospice patients in collaboration with the interdisciplinary hospice team. This role focuses on promoting comfort, dignity, and quality of life for patients and supporting their families throughout the end-of-life process.
Responsibilities:
Coordinate hospice care by collaborating with interdisciplinary team members, patients, and family members to ensure individualized care needs are met.
Evaluate the effectiveness of hospice services for patients and families on an ongoing basis and report findings to the care team.
Perform hospice evaluations and informational visits as needed.
Utilize the nursing process to deliver professional nursing care in accordance with hospice standards and physician orders.
Communicate with the attending physician and Hospice Medical Director regarding changes in patient condition that may require updated orders or modifications to the plan of care.
Complete and submit accurate, thorough, and timely clinical documentation.
Educate patients and family members about the dying process, the established plan of care, and available options to support informed healthcare decisions.
Provide nursing services, treatments, and preventative procedures requiring specialized skills as ordered by the physician.
Develop and prepare care plans for Hospice Aides to ensure continuity and quality of care.
Job Type: Part-Time
Pay Rate: $37–$40 per hour

QUALIFICATIONS
- Graduation from an accredited School of Practical Nursing.
- Must be licensed in NJ as a Licensed Practical/ Vocational Nurse by New Jersey State Board of Nursing in accordance with NJSA 45:11-23 et seq and NJAC 13:37.
- One or more years of nursing experience. Hospice experience preferred.
- Must be a licensed driver with an insured automobile in good working order.
- The ability to communicate well, both verbally and in writing.
