Description

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist works on determining a treatment plan, performing interventions to improve the patient's well-being, and evaluating progress.

Main Responsibilities:

Evaluates patient's functional status, body systems, and therapy needs.
Provides services within the scope of practice defined by state laws and in coordination with the healthcare team.
Observes and reports the patient's response to treatment.
Treats patients to relieve pain and restore function using evidence-based techniques.
Instructs the healthcare team, patient, and family on the plan of care.
Visits patients within the service area.
Completes progress notes and evaluations within guidelines.
Participates in conferences and committees.
May supervise home health aides and PTA.
Provides in-service education.
Adheres to therapy standards and maintains clinical knowledge.
Performs a weekend rotation for services.
Prepares discharge and summary reports.
Performs all other assigned duties.

Licensure Requirements:

Current Physical Therapy licensure in state
Current CPR certification
Must have a driver's license, vehicle insurance, and access to a reliable vehicle
