Social Worker Job Description:
The Social Worker is responsible for providing case management, discharge planning, and counseling services to residents and families of the Nursing Home. In addition, the Social Worker maintains contact with community agencies and individuals who can contribute to a comprehensive plan of care when appropriate.
Social Worker Job Responsibilities:
- Support the Social Services Director with department planning and organization necessary to ensure that the residents' medically related, emotional and social needs are maintained on an individual basis
- Develop and maintain good working rapport with residents, family members, interdisciplinary team members, service providers and facility staff to ensure that social services programs can be properly maintained.
- Participate in the admission and discharge planning processes, conducts resident assessments, care planning process and the coordination of resident room changes.
- Assist in maintaining and enhancing residents' abilities to manage daily physical and psychosocial needs.
- Connect residents and families with external resources, including financial assistance, legal aid, mental health support, and transportation services.
- Participate in planning for residents' admissions, discharge, or transfers to other facilities.
- Engage in interdisciplinary care plan meetings to ensure the social and emotional needs of residents are addressed.
Social Worker Job Requirements:
- Long-term care experience is a must
- Strong ability to manage, multi-task, and communicate effectively
