    Catholic Community Services Foundational Community Supports (FCS) program provides Supportive Housing and Supportive Employment Services under the Medicaid Transformation Project.

    These services are for people with complex behavioral and or physical health needs who benefit from additional supports in order to be successful in finding and maintaining housing or employment.

    FCS services are integrated within a range of programs across clinical teams, shelter and housing sites, and navigation services.

    The FCS team collaborates closely with staff and colleagues to develop and carry out care plans based on individuals unique needs and goals.

    Staff work with individuals to build rapport and develop trust in order to assist with:
    housing/employment search and placement, stability planning, community resource linkages, care coordination with providers, landlord-tenant mediation or employer advocacy.

    Because lack of or unstable housing and employment can have direct impacts on a range of health outcomes, Washington State Health Care Authority considers the FCS scope of work a health care service.

    Additionally, FCS Supported Housing and Supported Employment are considered Recovery Support services as part of Catholic Community Services of King Countys Behavioral Health Agency license, and so also must follow standards of Washington Administrative Codes (WAC) as monitored by the Department of Health.

    The FCS Program Director serves as an inspirational and insightful leader, responsible for all FCS Supportive Housing and Supportive Employment services provided in King County.

    This position oversees the quality provision of direct service and ensures that documentation is compliant with all federal and state regulations as well as funder requirements.

    Program revenue is directly impacted by inaccuracies or errors in data reporting or submission, and so oversight of accurate and timely documentation is critical to ensuring the programs fiscal solvency.

    This service integration involves hybrid staffing models in which supervision may be shared between FCS program leadership and a staffs primary program supervisor.

    The position maintains responsibility for updating, distributing and/or creating written program practice guidelines regarding data entry, enrollment eligibility, effective clinical documentation, Medicaid authorizations, and billing reconciliation.

    Maintain oversight of FCS service areas across the agency, monitoring the staffing arrangements and programs where the FCS benefit is being utilized.

    Work with other agency leadership to find ways to most effectively leverage this Medicaid funding source in housing, shelter, and navigation programs.

    Ensure that all FCS-involved staff receive adequate coaching and training to effectively and accurately provide FCS services, in alignment with program, funder, and federal requirements.

    Provide and oversee the direct supervision of FCS-involved staff.

    Monitor the goal planning and crisis response procedures among FCS case managers in order to promote safety of all direct service staff, enrolled individuals and involved community members.

    Provide cross-system troubleshooting and consultation with clinical staff, health care clinics, social service providers, housing and shelter staff and all other systems as needed for service development, community planning and resource linkages.

    Review reports and data dashboards regularly to evaluate data, assess strengths, note outliers and identify areas of improvement needed.

    Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).

    Comply with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).

    Provide direct supervision and back up support to the FCS Compliance Specialist, who is responsible for verifying FCS program eligibility, tracking authorizations as required for reimbursement, and reconciliation of claims.

    Act as a lead point of contact between the agencys FCS Program and the Medicaid payer to ensure that the operations of authorization and reimbursements are functioning appropriately.

    Attend county- and state-wide FCS program leadership meetings or workgroups, representing the agency in advocating for modifications to processes that improve the overall quality and functionality of FCS implementation across the state and application within programming.

    Work in partnership with regional leadership as appropriate in order to assist in the management of fiscal resources and promote goals of the (Federal) Integrated Healthcare Triple Aim; 1) highest quality services and satisfaction, 2) better physical and behavioral health outcomes, and 3) fiscal efficiency.

    Work with the teams to ensure that any unpaid claims are prevented as much as possible. Advocate with the program payer for full reimbursement of claims when denials and rejections appear to be inaccurate.

    Review and interpret monthly financials and trend reports for site in order to respond as needed to fiscal issues.

    Participate in solution-focused financial discussions with leadership and case management teams.

    Maintain a structured CQI process that incorporates regular chart reviews for Medicaid, state regulatory requirements, funder requirements, and contract compliance and quality.

    Ensure that the process in place affirms services provided have been ordered on the goal plans, per Medicaid requirements, and that strategies and treatments provided originate in the assessment, intake, treatment plans and are noted consistently throughout all documents (golden thread).

    Confirm funders (local, state or federal entities) receive documented completion of deliverables per contract requirements and timeframes.

    Review monthly data and fiscal management reports, identify areas of concern, and develop/implement a plan to correct data or financial issues.

    Evaluate and Monitor Site Performance
    Communicate effective standards for site-based operations including administrative and programmatic activities.
    Set clear and specific staff expectations for service hours, contract compliance, and employee conduct.
    Review staff service hours regularly, and develop solution-focused plans to aid staff in meeting program expectations.

    Assess staff performance and practice ensuring all annual reviews are completed and are accurately reflecting staff performance and work goals.

    Solicit feedback from involved stakeholders, community members, and agency employees on the benefits, impacts, and growth areas of the FCS program
    Recommend policy or protocol changes that are necessary to enhance FCS operations.
    System Innovation and Community Responsiveness

    Develop a skilled workforce, maximize retention, and incorporate innovative recruitment strategies, identifying specific needs of positions being filled and monitoring training and support needs of staff.

    Negotiate shared supervisory relationships with hybrid staffing models in which Case Managers work at a primary program (i.e. shelter or housing site) and a portion of their work duties involve FCS services.
    Promote partnerships between management, operations and direct service workforces.

    Facilitate routine meetings with other agency directors to ensure that there is consistency in how FCS case management staff are held accountable to program performance expectations.

    In partnership with regional leadership and colleagues, rearrange and strengthen staffing patterns as needed to continuously improve program performance and staff satisfaction as related to workload.

    Initiate new contracts, projects, and/or program activities based on community and agency need and innovate on existing practices to assure continuous learning and program improvements.

    Provide clinical support and guidance as needed across FCS programming in an effort to provide direct services in line with organizational and department values.

    Contribute to the training and staff development for FCS staff in a variety of mediums including individual coaching to group trainings.

    Address inter-staff conflict in a trauma informed and holistic way with an emphasis on relationship building.

    Bachelors degree in a social science OR five years of direct service in case management.
    Two years clinical experience working with adults who have complex behavioral health needs and living in homelessness.
    Washington State Department of Health Agency Affiliated Counselor registration or higher.
    Dynamic, flexible and creative thinker with ability to collaborate closely in shared supervisory structures of employees.
    Reliable transportation with ability to travel to different offices around Seattle and greater King County.
    Masters degree in Social Work, Psychology, or similar social service field.
