    Description

    Visits in-patients, assesses spiritual needs and provides sacraments and pastoral care to patients, visitors, and staff to facilitate the healing process.

    Attends interdisciplinary discharge planning meetings in order to be an integral member of the interdisciplinary team.


    Prepares and conducts various programs including employee and clergy orientations, Hospice volunteer training, and community education to make others aware of the scope of Pastoral Care at SEMC.

    May work North, South or Grant Cty Units and be on-call in order to provide pastoral care coverage for the entire Medical Center


    Maintains appropriate communications with other staff regarding patients by filling out Evening Report when working second shift, turning in weekly activities report, attending regular monthly staff meetings, and filing Patient Spiritual Assessment forms in patient charts or (for priests) signing chart when patients anointed.

    24 hour on-call coverage is provided by chaplains in order to meet the pastoral care needs of the patients. Priests are on-call to provide sacramental ministry.

    When working on

    Behavioral Health Units:

    completes initial spiritual assessment within 72 hours of patient admission; provides counseling as needed; gives lectures as part of overall program; works with interdisciplinary team and reports to program director.

    Provides prayer service and/or other group experiences in order to help patients understand the spiritual dimension of their recovery.

    When working with Hospice:

    communicates with patients clergy; makes introductory home visits; completes initial spiritual assessment on all patients; provides consultation/education to other interdisciplinary team members; supervises bereavement services; participates in interdisciplinary team conferences to enable all involved to communicate and receive support to deal with spiritual issues; reports to the Hospice directory/Nurse Manager.

    Performs other duties as assigned.


    Education, Credentials, Licenses:
    College graduate and ecclesiastical (denominational) endorsement

    Membership in the National Association of Catholic Chaplains or Association for Professional Chaplains

    Two units of Clinical Pastoral Education or equivalent


    Specialized Knowledge:
    Previous counseling experience


    Thorough understanding of the goals and objectives of SEMC including the Ethical and Religious Directives for Catholic Health Care Facilities.

    Knowledge of and respect for religious beliefs/practices of other faith groups.

    Two years pastoral ministry


    DESIRABLE
    Masters of Divinity

    Experience in specialized hospital units

    Certification by NACC or APC
