    Job Summary

    The Aftercare Specialist is responsible for aftercare services coordination and discharge planning for clients in PA Residential.

    Job Duties


    • Ensure compliance with expectations identified in discharge plan
    • Maintain ongoing contact and communication with county workers and aftercare referral sources to facilitate coordination of care
    • Document all contacts and aftercare services provided in client's record, and complete KidsPeace Discharge Information Sheets in EHR
    • Maintain a resource file on potential aftercare resource providers and act as a resource for treatment team members
    • Participate in Discharge Planning ITT meetings and act as a resource to the Clinician in the development of the discharge plan for the youth
    • Facilitate the setup of recommended aftercare services prior to the client's discharge from the program by completing referral packets for insurance companies and aftercare providers, scheduling of intake dates for each service including therapy, psychiatry, and schooling
    • Speak to family or caseworkers regarding agreement of recommended services and referrals
    • Coordinate psych evaluation requests with Case Managers to request funding updates and discharge recommendations
    • Weekly follow up with county when step down referrals are being made and assisting when necessary to make additional referrals or coordinate additional referrals for the county to submit
    • Oversight of the discharge planning process, ensuring disenrollment from EHR is complete and all necessary fields for KQI reporting are entered
    Qualifications


    • Associates Degree required. Bachelors Degree preferred
    • Experience in utilization management/quality assurance (preferably psychiatric) preferred
    • Experience in behavioral health, psychiatric field
    • Experience with acute emotionally challenged children or adolescents preferred
    • Must be at least 21 years of age
    • Valid driver's license required
