Practice Clinical Resource Nurse - Concord, New Hampshire

Job description
Summary
Reporting to the Practice Manager, and under the direction of the provider, delivers patient care including assessment, treatment, care planning, and medical care evaluation; provides training to clinical staff and is recognized as a resource to clinical and administrative staff.
Education
Two years of formal training or education beyond the high school level (e.g., Associates Degree).
Certification, Registration & Licensure
Licensure required: Registered Nurse Licensure through the State of New Hampshire Board of Nursing.
Certification required: Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers.
Experience
Minimum of one year as a Registered Nurse in a medical or acute-care setting.
Responsibilities
Coordinates educational and training programs for all clinical staff.
Develops and implements clinical protocols and work flows.
Performs both physical and psychosocial assessments.
Triages patient phone calls.
Documents into the patient electronic medical record.
Provides education to patients.
Administers prescribed medications.
Completes practice and unit specific clinical skill competencies.

The noise level in the work environment is usually moderate.
