Ovation Home Care, an award-winning private duty senior care agency, is actively seeking a Client Care Coordinator to provide the critical link between our office team and our caregivers where they serve our clients in residential and facility settings.
The Care Coordinator supports both our field staff (CNAs and Home Health Aides) as well as our clients and their families by making home visits 2-3 times per month. You must be CNA/HHA with 10+ years of experience to be considered.Duties:

• Field Visits.
• Work with other team members to coordinate various aspects of a client's start of care.
• Start of care "Meet & Greet".
• Recognize and pursue opportunities to promote client engagement.
• Assist Client Experience Manager with client related administrative tasks.
• Schedule and attend any necessary pre-discharge rehab training.
• Fill in for client shifts as needed.

• Complete and pass a Level 2 nationwide criminal background check, DMV check.
• Possess a valid driver's license and auto insurance with you listed as a covered driver
