    Overview

    We are actively recruiting a Pharmacist to join our Population Health Team. We are seeking a Pharmacist to be involved in the clinical management of CKD & ESKD patients.

    This candidate will be a critical member of the Population Healthcare team consisting of nurses, dietitians, social workers, community health workers, nurse practitioners and physicians.

    You will conduct medication reviews to identify and address medication discrepancies and improve care coordination for our complex renal patients with multiple chronic conditions and polypharmacy.

    You will also work with patients to provide medication education and tackle any financial concerns to medication costs.

    You will bring the subject matter expertise for clinical pharmacy, therapeutic alternatives, medication adherence strategies, immunizations, and specialty pharmacy to develop programs, policies, and workflows.

    Your primary focus will be improving patient outcomes by ensuring patients understand their medication regimens, optimizing medication plans, and reducing medication costs.

    This is a fully remote role but New Jersey, Licensure is required. Must be willing to cross license in Maine, Massachusetts, Rhode Island & Virginia.

    Responsibilities


    • Transitional care medication reviews.
    • Kidney care medication reviews.
    • Answer medication-related questions.
    • Conduct medication reviews.
    • Educate care team on medication-related topics (e.g., Adherence, side effects, affordability).
    • Outreach to nephrologists, primary care providers, and dialysis center staff for medication coordination.
    • Work collaboratively with care team members to address patient's social and medical needs holistically.
    • Assist in the design, development, and implementation of pharmacy specific programs and pharmacy components of other care management programs, for example understanding best practices, creating key metrics and documentation to capture key metrics, and toolkit development and refinement.
    • Review and analyze key medication metrics to understand performance, break down key drivers, and identify new opportunities.
    • Articulate various techniques of prescribing medications and issues related to the drug process.
    • Ability to train public health worker regarding pharmacological concepts Knowledge and evolve from a disease-oriented practice to a patient-centered focusing on prevention.
    • Knowledge of state laws, local, and federal regulations.
    • Collaborate with Primary Care Physicians (PCP) and nephrologist regarding medication management.
    • Ability to counsel patient and reduce health disparities through cultural competence and promoting diversity.

    Required/Preferred


    • Master's Degree in Healthcare Administration or Business or bachelor's degree in Pharmacy.
    • 3-5 years in clinical practice.
    • Analytical Skills.
    • Doctorate Degree in Pharmacy (PharmD).
    • Care Management Experience.
    • Demonstrates empathy, enthusiasm, a great sense of humor, and a strong work ethic.
    • Proactively acts as a patient advocate and responds with resolve.
    • Knowledge and experience to empower patients in self-management and shared decision making.
    • Experience working with a vulnerable patient population (ESRD, geriatrics, minorities, low income,
    uninsured, etc.).


    • Ability to establish rapport with patient and family by inquiring and listening.
    • Enjoys working collaboratively with team members.
    • Familiar with electronic medical records.

    EDUCATION, EXPERIENCE AND LICENSING REQUIREMENTS:

    • Graduate degree from an accredited pharmacy college recognized by the American Council of
    Pharmaceutical Education (ACPE).


    • Completion of an emergency training course approved by the American Council of Pharmaceutical
    Education (ACPE), curriculum-based programs from an ACPE-accredited college of pharmacy,

    applicable state or local health department programs, or programs recognized by the board of pharmacy

    upon hire.


    • Valid state pharmacist license (or must meet education requirement(s) for state licensure).
    • Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification upon
    hire.
