This position will be supporting Providence Transitional Care Center.Schedule: 36 hours/week, working 12 hour shifts
Job Description:
The Primary Care Nurse is in charge of the resident care for a specific group of residents, to include, supervision of team members, coordinating referrals to other members of the interdisciplinary healthcare team, families and physicians.
Providence Transitional Care Center has a total of 52 beds with a good mix of acute stay residents vs. Long Term Care. This unit is about stability and the work is fun and challenging. It has 12-18 Medicare beds.
