    This position will be supporting Providence Transitional Care Center.

    Schedule: 36 hours/week, working 12 hour shifts


    Job Description:


    The Primary Care Nurse is in charge of the resident care for a specific group of residents, to include, supervision of team members, coordinating referrals to other members of the interdisciplinary healthcare team, families and physicians.

    The Primary Care Nurse serves as the point of contact for resident information. Assists with clinical interventions and resident activities of daily living.

    Providence Transitional Care Center has a total of 52 beds with a good mix of acute stay residents vs. Long Term Care. This unit is about stability and the work is fun and challenging. It has 12-18 Medicare beds.

    Required qualifications:

    • Graduate of an accredited School of Nursing.
    • Upon hire: Alaska Registered Nurse License
    • Upon hire: National Provider BLS - American Heart Association

    Preferred qualifications:

    • 6 months of Nursing experience in a health care facility (acute care, long term care, rehabilitation center).
    • Supervision/leadership experience.
