- Responsible for the tracking, coordination, and communication of patient referrals.
- Ensure complete and accurate patient registration, including demographics and current insurance information.
- Handle requests from third party referral sources and provide relevant information and data.
- Serve as point person for multidisciplinary evaluation process to include patient scheduling, third party communications, and internal communications.
- Assemble information concerning patient's clinical background and referral needs.
- Contact insurance companies to ensure verification of benefits and prior authorization approval requirements are met.
- Review details and expectation of referral with the potential patient.
- Ensure that referrals are addressed in a timely manner.
- Communicate with referral sources to ensure patient information/registration is complete.
- Communicate patient updates between program and referring source.
- High School Diploma or equivalent
- Two or more years of experience with Electronic Health Record
- Strong customer service focus
- Effective verbal and written communication skills

CMADC is seeking an enthusiastic and organized individual to join our team as a Referral Coordinator. The referral coordinator will perform a wide range of administrative duties including referral processing, patient registration, verification of benefits, insurance authorizations, patient appointment scheduling, and communication with referral sources.
