    Our Integrated Care Hospitalist program is at the forefront of this mission, bridging the gap between primary care and inpatient medicine.

    As a Hospitalist, you will play a vital role in providing high-quality, personalized care to our patients.

    You will work closely with a multidisciplinary team to ensure that patients receive a seamless continuum of care from admission to discharge, focusing not only on their acute medical needs but also on their long-term health and well-being.


    Inpatient Care:
    Provide exceptional inpatient care for patients admitted to the hospital. This includes diagnosing and treating acute medical conditions, managing complex cases, and coordinating care with specialists.
    # Serve as a patient advocate, ensuring that the patient's voice and values are heard and respected throughout their hospital stay.
    # Care Coordination:

    Collaborate closely with primary care providers, specialists, nurses, social workers, and other healthcare professionals to ensure a coordinated and holistic approach to patient care.

    # Quality Improvement:
    Participate in quality improvement initiatives, research, and education to advance the standards of inpatient and integrated care.
    # Educate patients and their families on their conditions, treatment options, and preventive measures, empowering them to take an active role in their health.
    # Hospital Rounds:
    Conduct daily rounds, assessing patients' progress and making timely adjustments to their care plans as necessary.
    # Maintain accurate and comprehensive medical records, ensuring that all pertinent information is accessible to the care team.

    Medical degree (MD or DO) and BE/BC in Internal Medicine or Family Medicine.
    California State medical licensure and hospital privileges.
    Commitment to patient-centered care and a collaborative approach to medicine.
    A passion for improving healthcare systems and patient outcomes.
