    JOB SUMMARY:

    Provides physical therapy services to students with varying disabilities under the direction and supervision of a licensed physical therapist.

    Collaborates with all members of the IEP team and families to improve student outcomes.


    ESSENTIAL DUTIES AND RESPONSIBILITIES:

Plans and implements therapeutic programs, based on individual's goals and objectives, under the supervision of a licensed Physical Therapist. Daily 60% 2.

    Assists supervising Physical Therapist with instructing the educational staff concerning use of adaptive equipment, transfer and movement techniques, positioning program, maintaining equipment and integrating therapy goals into the learning environment.

    Daily 10% 3.

    Consults weekly with supervising Physical Therapist concerning implementing interventions, accommodations and modifications, student's response to treatment, and use of adaptive equipment.

    Daily 5% 4. Contributes to progress reports by collecting data and other documentation as requested by supervising Physical Therapist. Daily 5% 5. Completes Medicaid forms as directed. Daily 5% 6. Participates in IEP as requested and Physical Therapy meetings. Daily 5% 7. Organize, inventory, and maintain equipment. Daily 5% 8. Assist with clerical and logistical needs of the program. Daily 5% 13. Perform other duties as assigned or requested.

    This job has no direct supervisory responsibilities

    MINIMUM QUALIFICATIONS:

    The minimum prerequisites of formal education, professional work experience, certifications, licenses, endorsements, designations, trainings, and other criteria that a candidate must fulfill to be considered for a position.

    It is essential that certifications, licenses, endorsements, designations, and trainings are fulfilled, valid, and not expired. Criminal background check and fingerprinting required Associate's degree Completion of a two-year accredited program in Physical Therapist Assistant. Demonstrates effective communication skills (oral and written).
