Join our Compassionate Team:

We are currently seeking a Licensed Social Worker (LSW) to join our Social Service team.

Responsibilities as a Hospice Social Worker:

• Offering a broad range of psychosocial services to terminally ill patients and their loved ones
• Initiating contact with patients, families, and caregivers within 48 hours of admission
• Performing duties of a Case Manager
• Ensuring timely documentation of psychosocial assessments in patient charts
• Regularly evaluating the psychosocial and pre-bereavement needs of patients and families
• Implementing tailored care plans to address their social service goals and emotional needs
• Participating in interdisciplinary team meetings on a weekly basis
• Providing support in making funeral arrangements when requested
• Offering counseling throughout the terminal illness process and at the time of passing upon request

Required Skills:

• Qualifications for Social Worker Role:
• Bachelors Degree from a school of social work accredited by the Council on Social Work Education
• Minimum of two years' social work experience in a healthcare or social services setting
• Current Social Worker license
• Prior Hospice experience is advantageous
• Proficiency in identifying and obtaining community resources
• Sound understanding of Hospice philosophy and end-of-life principles
• Excellent organizational and communication abilities
