- Coordinate multidisciplinary patient centered care team approach for patient population
- Manage course of treatment for patients, coordinating care with physicians, nurses and staff
- Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes
- Ensure continuity of care through discharge planning and utilization of resources
- Collaborate with other departments and community health care agencies
- Participate in continuous quality improvement activities
- Graduate of nationally accredited nursing program
- Valid RN license in NM or allowed by reciprocal agreement by NM
- Current CPR/BLS certification for healthcare providers within 30 days of hire
- At least 1 year of directly related experience
- Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly
- May require walking or standing for significant periods of time or entail the constant pushing and/or pulling of materials
- Physical risks, dirt, dust, fumes, noise
