RN Case Manager

Sign-on bonus and relocation reimbursement available

• Coordinate multidisciplinary patient centered care team approach for patient population
• Manage course of treatment for patients, coordinating care with physicians, nurses and staff
• Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes
• Ensure continuity of care through discharge planning and utilization of resources
• Collaborate with other departments and community health care agencies
• Participate in continuous quality improvement activities

Required Credentials and Qualifications:

• Graduate of nationally accredited nursing program
• Valid RN license in NM or allowed by reciprocal agreement by NM
• Current CPR/BLS certification for healthcare providers within 30 days of hire
• At least 1 year of directly related experience

Physical Requirements:

• Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly
• May require walking or standing for significant periods of time or entail the constant pushing and/or pulling of materials
