Job description
Join a dedicated healthcare team providing exceptional post-acute care in the community.This full-time Advanced Practice Provider role involves managing patient care, conducting rounds, writing orders, and coordinating diagnostics, with oversight from a Medical Director.
Coordinate and deliver quality patient care while collaborating with staff and physicians.Maintain accurate documentation in electronic medical records and handle billing-related tasks.
Communicate effectively and provide compassionate, empathetic care.
Requires Nurse Practitioner license, DEA certification, and preferred experience in geriatrics, internal medicine, or related fields.

Standard day shift with on-call schedule; travel to Lynchburg, VA may be required.
