Job summary
The LPN Medication Care Manager is responsible for providing quality care by administering medication treatments in a safe organized manner reviewing reading notating initialing electronic health records documenting pertinent resident information receiving medication updates from Resident Care Director Wellness Nurse administering assisting observing medications treatments using medication administration record Six Rights of Medication Pass ensuring medications passed according to times utilizing mobile medication cart documenting initializing medications given ensuring appropriate documentation completed refusal missed doses maintaining confidentiality all resident information including resident medication reporting resident concerns unavailable medication RCD Wellness Nurse restocking medication cart checking medicatedependent packaging system assessing residents determine need as neededmedication appropriately document report supervisor counting narcotics another Medication Care Manager LeadCare Manager each shift reporting discrepancies RCD Wellness Nurermaintaining cleaningMedication Room treatment carts neatness cleanliness availability expiredmedications following refillprocess helpingresidents maintain independence promote dignity physical safety each resident adhering Sunrise Principles Service striving understand respond each resident empathy remaining mindful unique communication patterns history basic human needs practicing routinely good standardcare precautions cleanliness hygienehealth auditing medicatedepartments.
