    Job Description

    We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team for PRN Home Risk Assessments. In this role, you will conduct adult/geriatric assessments, work collaboratively with physicians and other healthcare professionals, and provide high-quality patient education and follow-up care.

    Responsibilities

    • Conduct comprehensive health assessments, including medical history, diagnosis, and treatment
    • Collaborate with physicians, case managers, social workers, and other healthcare professionals as needed
    • Provide patient education and follow-up care to promote optimal health outcomes
    Requirements
    • Master's Degree required OR commensurate experience and satisfactory completion of NP licensure
    • Current RN and NP or PA licensure in state of practice, including prescription authority or the ability to obtain prescriptive authority
    • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
    • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
