- Bachelor's Degree, experience in social work, 2 years of assessment history, care planning, communication skills, observations, evaluations, and be at least 18 years of age.
- Pre-Employment Requirements: Clear SLED Background Check, required documents, training, car insurance, TB Test, driver's license, 10-year MVR, and pass knowledge test.
- Monthly contact with Participant (various types of visits including face-to-face).
- 30% travel or more to Participants, meetings, and training using personal vehicle in a minimum of 1 entire area of 1 regional office.
- Time management, active listening, story teller, and problem solver.
- Prioritize tasks, create and terminate services, and work independently.
- Willingness to learn, receive constructive criticism, and make consistent improvement.
- Available from 8:30am - 5:00pm, Monday - Friday.
- Case Managers cannot have other employment for more than four (4) hours between 8:30am - 5:00pm, Monday - Friday.
