The Heart Institute of Venice is a patient-centered growing cardiovascular and primary care practice dedicated to delivering high-quality compassionate and evidence-based care to our community.We are currently seeking a skilled Advanced Practice Registered Nurse APRN with cardiology experience and a Primary Care Physician MD/DO to join our collaborative clinical team.
Responsibilities include conducting comprehensive cardiovascular assessments examinations diagnosing treating and managing acute and chronic cardiac conditions developing individualized treatment plans providing patient education on heart health medications lifestyle modifications collaborating closely with cardiologists multidisciplinary team members documenting accurately in EMR maintaining compliance with clinical guidelines participating in care coordination follow-ups providing comprehensive primary care services to adult geriatric patients diagnosing managing acute chronic illnesses performing preventive care annual wellness visits screenings developing long-term care plans coordinating specialty referrals maintaining accurate timely medical records collaborating with cardiology specialty teams for integrated care educating patients on preventative health disease management having excellent communication teamwork skills Qualifications include active Florida APRN license board certification ANCC/AANP minimum years cardiology internal medicine experience preferred strong clinical decision-making communication skills BLS/ACLS certification experience outpatient cardiology setting familiarity cardiac diagnostic testing procedures MD or DO degree accredited institution board certified eligible Internal Medicine Family Medicine active Florida medical license strong commitment patient-centered care excellent communication teamwork skills
