Job summary
The Transitional Care Support Health Coach (HC) is key to ensuring safe and effective transfers in the movement of patients across the care continuum. The HC assists patients with medication management, scheduling follow-up appointments with their physician/specialist, and learn to recognize symptoms that indicate their condition is worsening.Responsibilities
- Conduct telephonic or in-person hospital inquiries with Care Managers.
- Maintain accurate and timely documentation on each referred patient.
- Evaluate aspects of each patient's condition, diagnoses, medications and support systems.
