    Description
    Hastings Health and Senior Living Care Center *is hiring a *Part-Time Social Worker Designee* to join our team.

    In this role, you will work primarily advocate for our residents on the Long Term Care unit and the Transitional Care Unit (TCU) and work with the residents and family members to ensure they receive high-quality care and services.

    We have received the *2020 Pinnacle Employee Experience Award for achieving employee satisfaction in Orientation and Training*. We have also had a deficiency-free survey for 8 years

    This is a *Part-Time,* benefit eligible position working a *flexible schedule* that works for you *Provide support to residents and their families on medical, emotional, physical, or psychological concerns.


    • Conduct tours and assist residents with adjustment to their new surroundings.
    *PRN Job Duties:
    • Provide services by completing MDS's, CAA's, and Care conferences with families.
    • Works in conjunction with another full time social worker as well as an admissions coordinator.
    • Assist the team in coordinating social services for the facility, writing individualized plans of care for the residents, medical equipment, counseling or other follow-up treatment plans.
    • Commitment to compassion, excellence and quality care.
    • Act as liaison between families, staff, and various agencies.
    • Act as an advocate ensuring basic human and resident rights.
    • Participate in admissions, provide appropriate referrals and discharge planning of residents.
    • Provides resources and referrals to staff, residents, and family members.
    • Helps to develop individualized care plans for residents with behavioral symptoms.
    *Experience working in LTC or nursing home type environment preferred.

    • Must have knowledge of state and federal guidelines related to long-term care and resident rights.
