Physical Therapist PRN

We are seeking a passionate Physical Therapist to work with our elderly patients at Assisted Livings and Skilled Nursing Facility in Ft Wayne, IN.

Responsibilities:

• Evaluate and treat patients according to physician orders using therapeutic procedures to address physical deficits and improve independence, safety, and quality of life.
• Manage the delivery of care throughout treatment and delegate components of care to assistants under supervision.
• Participate as a member of the interdisciplinary team providing support and information within physical therapy.

Primary Responsibilities:

1. Review patient information and evaluate its impact on assessment and treatment.
2. Conduct and document thorough functional assessments using standardized procedures, including home/vocational and equipment needs.
3. Develop and implement individualized treatment plans based on patient needs and containing achievable functional goals.
4. Comply with evaluation, treatment, and documentation guidelines.
5. Record patient progress every 7 days and revise treatment plans as needed.
6. Document the course of treatment in a clear and concise manner, including a discharge summary.
7. Instruct assistants in treatment procedures and provide close supervision.
8. Orient patients to program services and treatment procedures.
9. Work with nursing staff, patients, and families to maximize patient use of functional abilities.
10. Establish and instruct patients and family members in exercises and therapeutic procedures to be continued after discharge.
11. Assist in referral processes when patients require additional services after discharge.
12. Attend rehabilitation team conferences and participate in other department meetings.

Requirements:

• Master's or Bachelor's degree in Physical Therapy and 5 years of direct practice experience.
• Current license/certification in the state of practice.
• Prefer 1 year experience in rehabilitation, preferably with geriatric patient population.
• Excellent oral and written communication skills.
• Adequate computer skills for required job duties.
• Demonstrated competency in applying principles, methods, materials, and equipment used in physical therapy.
