    POSITION FEATURES:

    Home Dialysis Case Manager/New Patient Trainer
    Paid personalized training and education provided
    Empower, engage, and educate dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment
    Primarily works in the clinic setting and conducts initial home visits with another team member present
    Position serves as an onsite Patient Educator and Case Manager for patients with various shifts and work hours

    PURPOSE AND SCOPE:
    The registered professional nurse RN CAP 1 is an entry-level designation into the Clinical Advancement Program (CAP). This role is accountable for providing clinically competent care to an assigned group of patients, including assessment, planning, intervention, and evaluation. As a part of the End Stage Kidney Disease (ESKD) healthcare team, this position engages in decision-making, teaching, leadership functions, and quality improvement activities to enhance patient care outcomes and facility operations.

    PRINCIPAL DUTIES AND RESPONSIBILITIES:
    All duties and responsibilities are expected to be performed following Fresenius Kidney Care policies, procedures, and standards of nursing practice.
    • Works under the direction of the Supervisor or in collaboration with another Registered Nurse
    • Collects and analyzes dialysis data, makes adjustments to treatment plans, and notifies Supervisor or physician as needed
    • Assesses and documents patient/family's learning needs for ongoing education
    • Directs and provides patient care staff with safe and effective dialysis therapy delivery
    • Administers medications as prescribed and documents appropriately
    • Assists with emergency response measures when necessary
    • Serves as a resource for the healthcare team, participates in staff training, and ensures correct laboratory procedures
    • Identifies expected outcomes, updates the nursing assessment, and plan of care
    • Ensures patient awareness regarding treatment options
    • Assists in the evaluation and education of Home Dialysis candidates and partners
    • Performs home environment assessments for Home Dialysis candidates
    • Trains Home Dialysis patients and partners on equipment operation and maintenance
    • Completes CAP requirements for advancement
    • Performs other assigned duties by Supervisor

    PHYSICAL DEMANDS AND WORKING CONDITIONS:
    The role involves direct patient care, including heavy lifting and standing for prolonged periods. May require assistance for moving equipment and lifting.
    • Occasionally moves heavy machines and equipment, lifts weight, and may be exposed to infectious materials
    • Works in a healthcare facility with controlled temperature and moderate noise levels
    • Rotates coverage with other staff, participates in on-call rotation as needed
    • May require travel to training sites or patient homes
    • Engages in desk work, computer tasks, and interaction with patients and staff

    SUPERVISION:
    Oversees LPNs/LVNs, RNs, Patient Care Technicians, and Assistants as designated Nurse in charge
    • Must meet specific education and training requirements
    • Requires minimum experience as a RN and in home dialysis

    EDUCATION and LICENSURE:
    • Graduate of a Nursing School
    • Current state licensure and CPR BLS Certification
    • Meets practice requirements in the employing state

    EXPERIENCE AND REQUIRED SKILLS:
    • Entry-level for RNs with Nephrology Nursing experience
    • Minimum of 1-year experience as a Registered Nurse (preferred)
    • Experience in home dialysis therapy (preferred)
