    We have two (2) openings for LPNs to provide dedicated care to a patient in the 38106 area.

    You'll work on a rotating schedule (week 1:
    Mon-Tues-Sat-Sun, week 2: Wed-Thurs-Fri), offering consistent in-home support.
    Value building relationships with patients and families
    Current LPN license and basic care skills. We welcome experienced LPNs and passionate new graduates

    Join Adoration Health and build a fulfilling career path
    Perform selected nursing procedures within the scope of LPN practice
    Report significant changes immediately
    Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
    Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
    Cooperate with on-call schedule, weekends and after hour visits, as necessary
    Previous experience in home health, hospice, hospital or skilled nursing preferred


    • Familiarity with electronic health records
