    Registered Nurse - Home Health

    Secure Care Home Health Registered Nurse (RN) is responsible for the provision of skilled nursing services to patients in the home setting. The registered staff nurse reports directly to the Director of Nursing.

    Responsibilities and Duties:

    • Performs start of care visits, initial assessments, routine visits, and re-evaluations as assigned
    • Develops, implements, and maintains the nursing care for assigned patients under the direction of the physician
    • Initiates preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety
    • Observes and reports symptoms, reaction to treatments, drugs, and changes in the patient's physical or emotional condition.
    • Assures that progress reports are made to the physician for patients under medical care describing patient activities and/or changes in condition
    • Counsels the patient and family in meeting nursing and related needs
    • May assign selected portions of patient care to the Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA) or Home Health Aide (HHA) but always retains the full responsibility for the care given and for supervisory visits to the patient's home
    • Completes documentation at the time of the visit
    • Participates in on-call rotation
    • In-service events, formal and informal education events to insure delivery of Secure Care and Alivia Care Solutions messages
    • Maintain accurate record of all expenses and report to Secure Care on a timely basis
    • Exhibit care in complying with government mandated limits on amount and type of expenses related to Referral Partners
    • Collaborate with other members of the in-home care team within your assigned territory
    • Maintain memberships in related health care professional organizations for the purpose of networking
    • Communicate issues that arise in the field with clinical staff through defined channels to enhance service delivery
    • Participate in organizational committees as identified that enhance the working relationships between departments within Secure Care
    • At all times, represent Secure Care professionally through care and/or services provided to all clients and patients
    • Comply with Secure Care and Alivia Care, Inc. policies, procedures, and standard practices
    • Maintain the confidentiality of patients and families within Secure Care as per government mandates
    • Use resources in a fiscally responsible manner
    • Promote Secure Care through participation in community and professional organizations
    • Improve own professional knowledge and skill level
    • Additional duties, as assigned
    Qualifications and Requirements:
    • Bachelor's degree in Nursing from an accredited School of Nursing or equivalent, preferred
      • ASN Certificate is acceptable with appropriate experience
    • Current Licensure as a registered nurse in the state of Florida
    • Current State of Florida driver's license with driving record acceptable to Alivia Care insurance carrier, and reliable transportation
    • Current Auto Insurance coverage
    • Current CPR certification
    • Level II ACHA (Agency for Healthcare Administration Background Screening
    • Bi-lingual Spanish a plus
