    Key Responsibilities:

    • Conduct thorough risk and needs assessments to identify suitable substance abuse treatment options for clients.
    • Provide referrals to the appropriate level of care based on assessment findings.

    Scope of Duties:

    • Assess new clients using approved agency tools to determine counseling needs and recommended levels of care.
    • Develop personalized service plans that incorporate client-centered, recovery-oriented approaches.
    • Obtain relevant prior records to inform current treatment recommendations.
    • Monitor and document client progress according to funding source requirements and operational policies.
    • Regularly review, reassess, and revise service plans in collaboration with clients to address ongoing challenges and determine continued services or discharge.
    • Schedule and facilitate individual sessions as needed.
    • Identify and develop resources, connecting clients with necessary services.
    • Collaborate with referral sources and linked resources through active case consultation and coordination.
    • Organize and facilitate workshops, programs, and groups tailored to clients' specific needs.
    • Complete training agreements and fulfill obligations related to required treatment interventions.
    • Develop comprehensive discharge plans for all assigned clients, adhering to contractual requirements.
    • Participate in internal and external Treatment Team meetings.
    • Prepare and submit all necessary reports and documentation.
    • Demonstrate expertise in program-related treatment modalities.
    • Interact with clients, staff, and outside contacts with respect and professionalism, maintaining confidentiality in accordance with administrative policies.
    • Attend and engage in regular staff meetings, supervisory conferences, and training sessions.
