We are looking for a Night Shift Advanced Practice Professional to join our expanding team of Hospitalist Providers. The Hospitalist APP will work in conjunction with and support the Hospitalists in providing expeditious, safe, patient centered care.


Job Summary:

Responsible for providing direct patient care and supervising care of patients admitted to the Hospitalists# service.# Round on patients daily Provide patient and family education Deliver patient care for a variety of patients diagnoses utilizing evidence based practice.

Responsible for prompt recognition of acute change in patient status necessitating emergent change in plan of care.# Patient care includes performing detailed history and physicals on admission, rounds independent of physicians, diagnosing and treating patient with current scope of care, collaborating and consulting multidisciplinary team to optimize patient care and outcomes.# Order consults to specialists when deemed necessary.

Work closely with ancillary departments to ensure appropriate and timely work up and disposition of all patients. Provide comprehensive discharge plan to facilitate proper transition of care.

Serves as liaison between ED physicians, specialists and Primary Care physicians to coordinate patient care services in order to optimize care and maximize utilization of resources.

# Schedule:

Night Shift: 7pm-7am, 7 days on / 7 days off

Requirements:
Advanced clinical degree Valid NJ State Board P.A. or A.P.N. license, CDS and DEA Patient clinical management skills Excellent communication skills including active listening, verbal and written communication Experience and comfort working with physicians one-on-one and in small groups, including discussing clinical data and evidence-based treatment plans

Experience:

Required:
A minimum of 2 years# clinical experience in an acute care setting

Preferred:

Experience in one or more of the following: Hospitalist Program, Critical Care, Emergency Department Additional Information # ### To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
