As a Discharge Planner your responsibilities will include:
Provides discharge-planning activities for clients to include assessment of strengths and needs, services planning services brokering and client advocacyParticipates with team members in daily program activitiesCoordinates discharge planning and appropriate aftercare services for assigned clientsPrepares written documentation for each service conducted RequirementsNew Horizons will need you to have the following qualifications:Bachelor's Degree in Human related services field
