Job summary
This position provides patient navigation services to clients with health-related needs by coordinating access to available resources and providing informal counseling, education and social support. The Patient Navigator will work closely with external referral sources and internal departmental team members to ensure seamless service delivery. A strong working relationship must be maintained with these contacts in order for the navigator to provide quality service.
