Demonstrates the NM values of Patients First Integrity Teamwork and Excellence Presents a friendly approachable professional demeanor appearance Provides accurate information timely updates to patients customers Uses effective service recovery skills to solve problems or service breakdowns when they occur Communicates effectively with others respects diverse opinions styles acknowledges assistance contributions others Uses organizational unit department resources efficiently Facilitates collaborative care with other healthcare team members Manages work schedule efficiently completing tasks assignments on time Contributes opportunities processes for continuous improvement Participates in efforts reduce costs streamline work processes improve grow services we provide Demonstrates essential skill competencies selecting appropriate evaluation use treatment modalities equipment protocols Performs re-evaluations according to department guidelines adjusting treatment plan accordingly Evidenced by chart review Demonstrates creative problem solving diversity adapting treatment special circumstances Assumes complete responsibility OT patient management from time initial contact discharge functional progress safety treatment outcome as evidenced team feedback documentation Maintains primary responsibility patient plans care through timely consistent collaboration treating COTA(s) Adheres department policy regarding evaluations progress notes discharge summaries All notes reflect medical necessity accurately entered per policy electronic medical record Completes daily notes reflecting patient response treatment actual performance assessment status relevant treatment plan eventual discharge needs as evidenced chart review meeting all regulatory documentation requirements e.g allows other therapists continue treatment allows interpretation payers supports reimbursement Provides educational materials methods appropriate patient level understanding motivation as evidenced chart review observation team feedback Participates committees department processes special projects clinical expertise individual performance plan Creates culture respectful communication listening asking input open honest conversation approachability timely acknowledgement individual team contributions Works develop maintain professional relationships members healthcare teams e.g physician nurse case manager Participate internal external marketing per department expectations Accepts constructive feedback able make timely changes evidenced written coaching forms/chart audits team feedback Proactively completes non-patient care responsibilities established time frames consistently e.g coverage notes PTO/schedule change requests bonus pay forms discharge checklists follow up phone calls Contributes educational efforts department/clinic e.g in-services Journal Club new hire orientation process improvement Actively participates student clinical observation programs serving clinical instructor mentor observation host per department expectations Maintains productivity standards per department expectations Completes all NM required education competencies training within required timeframes Demonstrates willingness accommodate patient care needs staff meetings specific meetings outside normal work hours}Responsibilities
