- Conducting comprehensive assessments for new members and bi-annually thereafter, developing and updating member management care plans accordingly
- Prescribing diagnostics and interventions to prevent unnecessary transfers or admissions
- Collaborating with various stakeholders to determine medical necessity for requested services
- Educating members and their representatives on managing disease processes and promoting wellness
- Providing support and education to facility staff, among other responsibilities
