    Objective:

    This role involves providing medical care and management to program members and assisting in gathering clinical information for medical necessity assessments for service requests. You'll collaborate with various stakeholders to develop and implement medical management policies, clinical protocols, and quality management programs aimed at enhancing the health outcomes of members.

    Primary Responsibilities:

    • Adhering to state laws and regulations governing advanced practice nursing
    • Complying with long-term care policies, including those for nursing facilities and assisted living communities
    • Building and maintaining effective relationships with members, legal representatives, facility staff, PCPs, and other healthcare providers
    • Conducting comprehensive assessments for new members and bi-annually thereafter, developing and updating member management care plans accordingly
    • Prescribing diagnostics and interventions to prevent unnecessary transfers or admissions
    • Collaborating with various stakeholders to determine medical necessity for requested services
    • Educating members and their representatives on managing disease processes and promoting wellness
    • Providing support and education to facility staff, among other responsibilities

    Required Qualifications:

    • Completion of a Nurse Practitioner accredited program
    • Current state RN license
    • Licensed as NP in the state of practice
    • Knowledge of long-term care and patient care delivery systems

    Preferred Qualifications:

    • 2+ years of clinical experience
    • Familiarity with UM standards and regulatory guidelines
