    In this role, you will be essential in assisting our members to optimize their medication use, ensuring they are taking the right medication at the right time. Your efforts will contribute to improved clinical outcomes through better medication adherence and comprehensive chronic disease education. You'll collaborate with both patients and their healthcare providers to navigate the complexities of medication therapy management.

    Your Key Responsibilities:

    • Pharmacist Consult Services: Work directly with patients to manage chronic conditions; develop educational materials to empower patients; clarify benefits for better health outcomes.
    • Medication Therapy Management: Conduct thorough medication reviews; identify and resolve drug therapy issues to improve patient care and outcomes.
    • Quality Programs: Achieve top performance on pharmacy-related measures; recommend initiation or modifications of drug therapies; promote comprehensive medication reviews.
    • Practice Operations: Support on-call pharmacy needs; provide training on key pharmacotherapy topics; analyze drug utilization patterns and propose necessary interventions.

    Required Skills and Experience:

    • A state-licensed pharmacist with a minimum of 3 years of direct patient care experience.
    • Valid licensure in any of the following states at the time of application: Florida, Texas, Colorado, North Carolina, Pennsylvania, and Alabama.
    • Must obtain licensure in at least 5 states within 6 months of employment; must maintain a license in good standing.
    • Availability for travel as needed for licensure purposes.

    Preferred Attributes:

    • Experience with collaborative pharmacy practice agreements.
    • Board certification or specific experience in complex geriatric care.
    • Fluency in Spanish is highly desirable.
    • Willingness to provide coverage during Pacific and Mountain time zone business hours.
