Nurse Practitioner or Physician Assistant - Skilled Nursing Facility

• This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for longterm nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social wellbeing through personalized clinical care and therapies. They are currently hiring due to growth.

The APP is responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.

The APP conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients goals and needs of care.

The APP is supported by a Clinical Manager and Clinical Director, as well as a local Medical Director.

- interpret laboratory and other diagnostic data and recommend treatments or actions
- work collaboratively with physicians, care coordinators, facility staff, family members, etc.
- previous facility-based (SNF preferred) experience
