    Description

    Works with the Department of Family and Community Medicine, the Lewis Katz School of Medicine, and other stakeholders to coordinate various community outreach programs in the areas surrounding Temple University Hospital.

    Serves as a clinical resource to the community outreach team, triaging clinical concerns and creating protocols for various community-facing initiatives.

    Provides individual health-related education and coordination of care for program participants. Collaborates with each participants primary care provider. Supervises other members of the community outreach team to include medical assistants. Positively represents Temple University Health System in the community.

    Treats all participants with consideration to age, race, gender, gender identity, sexual orientation, religion, socioeconomic background, and culture in a caring, ethical, and professional manner.

    Education


    Masters Degree :
    Nursing (Required)

    Experience

    3 Years experience as a nurse practitioner (Required)

    5 Years clinical office experience (Required)

    Experience with community outreach (Preferred)

    General experience with EPIC EMR (Preferred)

    License/Certifications

    CRNP - PA Cert Reg Nurse Practitioner (Required)

    RN-LIC - PA Registered Nurse License (Required)

    PRX-AUTH - Prescriptive Authority (Required)

    BLS - Basic Life Support (Required)
