    Description

    This role would involve working in collaboration with the Medical Director, Program Director, Clinical Coordinator and HBOT Technician to provide quality patient care and assists the Medical Director in the medical leadership of the center.

    He/She will support the strategic growth of the center by promoting the services offered by the clinic to the local medical community.

    ESSENTIAL FUNCTIONS:
    The Wound & Hyperbaric Center is located within Hutchinson Regional Medical Center and operates as a Hospital Outpatient Department.

    We have a team of providers and healthcare professionals who provide advanced wound care for chronic and difficult to treat wounds.


    • Hours of operation for the Wound & Hyperbaric Center is Monday to Thursday 8AM to 4PM and Friday 8AM to 12PM.
    • Complies with all Kansas and JCAHO statutes and administrative regulations including the maintenance of a collaborative agreement with a supervising physician.
    • Must be comfortable performing appropriate advance wound care procedures such as various forms of debridement, tissue, cell- based skin substitutes and so on.
    • Activities include assessing health, education and psychosocial needs. Designing individual plan of care with patients and fostering a team approach by working with patient, family, primary care provider and other members of the health care team.
    • Provide Wound Care, Medication Management, Patient Education, Care Coordination, Physician Coordination, Personal Health Record and Discharge Services. Activities include implementing clinical interventions based upon risk stratification and evidence based guidelines in collaboration with the patient's Wound Care physician and/or primary physician.
    • Completes post discharge and post physician office visit medication reconciliation; supports patient in developing in home plan for medication acquisition, disposal, storage and administration; monitors adherence to prescribed medication regimen.
    • Trains patient in key self-management strategies using tool appropriate for patient's health literacy, cultural background, and level of activation. Activities include training patient to self-monitor to include the understanding of staging of condition status and appropriate action for each condition status. Tests and reinforces patient's learning and assimilation through Teach back or other agreed upon approach to education.
    • Ensures patient has a physician follow up visit post discharge; patient attends post-acute appointments, problem solves with patient to address barriers such as scheduling or transportation. Coordinates with program team members for removal of barriers.
    • Assists patient in establishing personal health record and provides ongoing support.
    • Assists with diagnosis, treatment and management of acute and chronic wound types.
    • Consults with wound care physician, designee or referring physician as indicated to discuss ordering of lab or radiology, interpretation of results, prescribing of medications, or other treatments, therapeutic or corrective measures.
    • Provides direct care and/or symptom management to wound patients as indicated in the absence of the Medical Director and assists in coverage of face to face assessments per regulatory requirements.
    • Prepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement for services provided. Performs documentation that is timely, concise and accurate.
    • Works in collaboration with the Clinical Documentation Improvement team to ensure improved patient outcomes, data quality and accurate reimbursement.
    • Assists in the development of clinical practice guidelines/standards in support of quality wound care.
    • Performs efficiently in crisis situations following established procedures, remaining calm, informing appropriate personnel and documenting events.
    • Participates in all mandatory professional staff meetings and continuing education sessions.
    • Establishes and maintains positive working relationships with patients, family members, program and agency staff, physicians and referral sources.
    • Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
    • Other duties as assigned.

    MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

    • Knowledge of MS Office software.
    • Keyboarding skills.
    • Demonstrated ability to communicate effectively.
    • Multitasking and prioritization skills.
    • Ability to apply advanced nursing principles.

    PREFERRED KNOWLEDGE AND SKILLS REQUIRED:

    • Clinical nursing experience in Chronic Wound Care or other relevant specialty
    • Chronic Disease Management/Educator
    • Case Management Certification

    MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

    • Physician Assistant

    PREFERRED EDUCATION AND EXPERIENCE:

    • Successful completion of a recognized introductory hyperbaric medicine training program as established by either the American College of Hyperbaric Medicine or the Undersea and Hyperbaric Medical Society.

    REQUIRED LICENSE/REGISTRATION/CERTIFICATION:

    • Current licensure as an PA in the State of Kansas.
    • NPI Number and DEA Number
    • Healthcare Provider BLS.
    • Workers' Compensation Insurance

    PREFERRED LICENSE/REGISTRATION/CERTIFICATION:

    • American Board of Wound Management (Certified Wound Specialist)
    • 1-2 years of Advanced Wound Care experience
    • 1-2 years of Hyperbaric Medicine experience
