Position Details
- Title: MDS Coordinator (LVN)
- Schedule: Part-Time
- Pay Range: $35.00 – $42.00 per hour, depending on experience
What You'll Do
- Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations
- Ensure care plans reflect resident needs, clinical status, and regulatory requirements
- Collaborate with interdisciplinary team members to support quality outcomes and reimbursement accuracy
- Monitor documentation for completeness and compliance
- Support survey readiness and quality improvement initiatives
- Serve as a clinical resource related to MDS processes and best practices
What We're Looking For
- Current, active LVN license in good standing
- Prior experience with MDS coordination in a skilled nursing or long-term care setting
- Strong knowledge of Medicare, Medicaid, and RAI processes
- Excellent organizational skills and attention to detail
- Ability to work independently while collaborating with a multidisciplinary team
- Strong communication skills and a positive, solutions-focused mindset
