Evergreen Village at Fort Wayne is seeking a Director of Nursing (DON) to join our teamResponsibilities:

Conducts initial assessment of prospective residents, including off-site visitsPerformswellness assessments and develops individualized care plans based on the needsand preferences of each residentDelivershealth promotion services, offers disease prevention counseling, and providesself-care education to residentsEngageswith residents and family members in a compassionate and impactful manner,creating a lasting and positive experienceFosterscommunication by regularly engaging with physicians, pertinent healthcareproviders, and family membersHires,trains, schedules, and leads the clinical team in providing exceptional careexperiencesCoordinatesthe Communitys infection control program while maintaining state regulatorycomplianceDevelopand oversee the Communitys medication management programContributes toward quality assurance and performance improvement initiativesComplies with all rules and regulations regarding confidentiality and privacy of resident informationQualifications:Maintainscurrent state nursing licensure, RN preferredMinimumof two (2) years nursing supervisory experiencePriorsenior/supportive living experience strongly preferredExcellentwritten and verbal communication skillsMustpossess leadership and supervisory ability and the readiness to mentor,motivate, and inspire team membersDemonstratesexceptional interpersonal skills and ability to serve with love, compassion, and dignityBenefits& Beyond:GrowthopportunitiesSelf-servicepayroll (PayCom)Medical,dental, and vision benefit packages availableDailypayWork-lifebalance | Paid Time Off401(k)employer match biweeklyEAP& Maven family planning programGardant Management Solutions proudly oversees the operations of this Community.
