    Description

    Pride Health is seeking a Physical Therapist to join our team in West Branch, MI This is a 13-week contract with potential for extension.

    Job Responsibilities:

    • Participates in the development of patient physical therapy plans of care.
    • Makes visits to the Patients for assessments and evaluations and communicates with the physician before start of care as needed.
    • Implements the plan of care.
    • Documents the care and submits paperwork in an accurate and timely manner to update and maintain the Patient medical records.
    • Provides education and instruction to team members, Patients, and their families.
    • May participate in agency quality assessment performance improvement (QAPI) programs.
    • Ensures the growth & profitability of the company through the responsible use of company resources and educating the community to our services.
    • Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the Patient and family, exchange information & problem solve & receive staff support and education.
    • Applies the policies and procedures of the agency and the rules and regulations of federal and state regulatory agencies and other certified agencies in providing care.

    Required Qualifications:

    • Bachelor's or Master's Degree in Physical Therapy.
    • Current state license to practice as a Physical Therapy Assistant.
    • Current BLS certification.
    • Minimum of 2 years of experience as a Physical Therapy Assistant.
    • Must have Home care Home base Experience.
    • Floating between departments and or local locations within 20 miles is required.

    Additional Information:

    • Location: West Branch, MI 48661
    • Schedule: 40 hours/week – Days – Monday – Friday 8AM – 5PM
    • Contract Length: 13 weeks
    • Gross weekly: $ 2,295.20(Candidate must live 60 miles away)
    • W2 hourly (Taxable): $26.23
    • Stipend Weekly (Non - Taxable): $1246.00
    • Local pay: $50 - $52 (For candidates who live within 60 miles)

    *Offered pay rate will be based on education, experience, and healthcare credentials.
