    Position Summary:

    The Stroke and Sepsis Coordinator is an expert nurse who provides leadership for the organization of services and systems necessary for a collaborative approach to stroke and sepsis care. The Coordinator plans, implements and evaluates clinical programs to achieve high quality, patient-focused outcomes which advance patient and family care and clinical team in stroke and sepsis care.

    ESSENTIAL FUNCTIONS

    Assists in the development and delivery of stroke and sepsis continuing education programs. In collaboration with Stroke and Sepsis Medical Directors, will help plan, coordinate and evaluate Stroke and Sepsis education for patients, provider, nursing and families related to the care and management of patients.Drag Edit DeletePromotes marketing efforts for the Stroke and Sepsis Program to internal and external audiences that will support the program image to the community.Drag Edit DeleteServes as a resource to staff and external contacts on issues related to patient care, patient flow and clinical standards, policies and procedures.Drag Edit DeleteMonitors departmental compliance with applicable requirements, including accreditation, legal, and The Joint Commission.Drag Edit DeleteCoordinates the development, implementation, review and revision of Stroke and Sepsis standards, policies, procedures and protocols.Drag Edit DeleteIncorporates knowledge of AHA Guidelines as well as emerging science and evidence-based practice into Stroke program standards.Drag Edit DeleteParticipates in Acute Stroke Code activations (when available) to ensure protocols are followed.Drag Edit DeleteLeads and participates in performance process. Assists in coordination of the Telestroke Program. Monitors Operational efficiency of the Stroke and Sepsis Program.Drag Edit DeleteParticipates in educational opportunities appropriate to professional goals. Remains current in research related to the stroke and sepsis patient population sand systems of care. In cooperation with Medical Staff, will interpret and apply current research into clinical practice. Actively participates in local, regional, and national advisory groups to advance the of care the person experiencing stroke and sepsis.Drag Edit DeleteWith support of the Clinical Team, crates and implements delivery models for the transitions of complex patients between multiple specialty and primary care providers and between healthcare settings and home. Evaluates critical pathway time frames, patient outcomes and identifies variances and corrective action(s).Drag Edit DeleteMaintains required competency levels as identified in written exams, skills, checklists, skills labs, annual safety, and health requirements as well as service excellence education hours requirements. Understands and adheres to compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflicts of Interest Policy.

    Minimum Education
    Associate's degree in Nursing
