PRN/On Call or As Needed - Delivers basic patient care including, but not limited to, comfort and care, vital sign monitoring, personal hygiene, and assists with mobility, including unit-specific tasks.
- Serves as a link between patients and nurses to communicate changes and/or issues.
- Provides exceptional customer service to all patients, their families, visitors, and staff.
- Ensures precise and timely documentation whenever necessary for patient care.
- Participates in the exchange/reporting of crucial information regarding patient and departmental needs when starting and finishing a shift.
- Executes assigned duties once competency has been verified.
- Assists patients using appropriate transfer techniques to promote autonomy and safety.
- Encourages patients to complete tasks as independently as possible to foster self-sufficiency.
- Tailors care based on the patient's hobbies and preferences.

- Must have completed one of the following: (required)Successful completion of nursing aide training program ORSuccessful passing of STNA State Tested Nurse Aide (required if OH LTC), OR CNA Certified Nurse Aide, OR Nurses Aide state testing or completed a nursing program that had one clinical nursing course and one clinical rotation.
- Recent experience in acute care, physician's office, home health/hospice, or long-term care facility, with a preference for the setting in which they are applying (preferred, not required)
- Possesses problem-solving skills with excellent communication and interpersonal skills
- Engages with staff and patients professionally
- Ability to be proactive in a fast-paced environment

Job Summary:
A **Patient Care Technician (PCT)** offers fundamental healthcare and assistance to patients under the supervision of a Registered Nurse or designated healthcare professional.
As part of our team, the **Patient Care Tech (PCT)** is responsible for maintaining the cleanliness of patients' rooms, monitoring patients' well-being and vital signs, as well as aiding patients with essential tasks such as feeding and grooming.
