    Description
    Transportation Aide
    MUST have an active CNA license, CPR Certificationand Valid Driver's License
    Responsible forcoordinating transportation and transportation of residents to and fromphysician appointments in a timely manner.
    The ideal candidate will be a dynamic self-starter and willhave excellent communication and organizational skills. Lee's Summit Place is a60 bed Rehab to Home facility with an emphasis on providing outstandingcustomer service and care.     Requirements:
    • A valid driving license with good driving record.
    • Knowledge of traffic rules and regulations
    • Sound judgement and decision-making
    • Excellent driving skills
    • Proficiency in using navigational software to map routes
    • Time management and organizational skills
    • Ability to communicate with passengers and clients
    • Must be able to pass a pre-employment background check
    Principal Responsibilities

    • Organizes transportation of residents to complete requiredappointments based on priority of resident needs.

    • Transports resident to and from inpatient and outpatientprograms and to and from appropriate facilities and residences.

    • Develops driving schedule based on resident's scheduledmedical appointments and program destination depending on resident appointmentneeds; Provides transportation of current or new residents from hospital afterdischarge.

    • Provides report, information, and assessment of resident'sbehavior or change of condition during transportation upon return to center;Reports changes of abnormal findings in resident status such as consciousness,pain, bleeding and vital signs that occur during the lift or transporting;Observe residents carefully and reports changes in condition to clinicalsupervisor.

    • Maintains a high level of safety following safety policiesand procedures.

    • Maintains van in a neat and clean condition at all times;Maintains van in good working condition; Ensures that routine maintenance onvehicle is performed, logs mileage, and that vehicle is clean.

    • Assist clinical staff with safe lifting, transferring,and/or transporting residents as specified by the physician orders and/orclinical supervisor; Assist in the care of residents requiring moderate tototal assistance in moving.

    • Establishes positive communications with outsidefacilities, hospitals, physicians' offices, and /or specialty clinics to ensureproper services for residents are maintained and scheduled appropriately.

    • Promptly reports any scheduling changes to the DON;nursing staff and ancillary service providers, responsible parties and takes follow-upaction as necessary.

    • Provide adequate hydration to residents at regularintervals each day according to specific resident needs.

    • Protect the personal property of the residents of thefacility.
