Director of Social Work

Bronx, NY

Job Summary:

We are seeking a compassionate and experienced Director of Social Work to join our team in Bronx, NY. As a Director of Social Work, you will be responsible for directing, establishing, and planning the overall policies and goals for our Social Services Department. You will oversee assessments, care planning, and helping residents adjust to our post-acute care continuum. If you have a strong background in social work, exceptional interpersonal and leadership skills, and a passion for providing top-notch care, we encourage you to apply.

Responsibilities:

• Directs, establishes, and plans the overall policies and goals for the Social Services Department

• Responsible for assessments, care planning, and helping residents adjust to our post-acute care continuum

• Supervises Department Staff, ensuring exceptional care and support to our residents

• Conducts discharge planning at the start of admission, ensuring a smooth transition for our residents

• Participates in occasional community outreach activities to promote our services and build relationships with local communities

• Demonstrates a strong understanding of various concepts, practices, and procedures in social work

• Works independently, relying on extensive experience and judgment to plan and accomplish goals

• Utilizes a wide degree of creativity and latitude to develop innovative solutions and initiatives

Requirements:

• 2+ years of management experience in long-term care settings

• LSW or MSW degree

• Strong knowledge of applicable State and Federal guidelines

• Exceptional interpersonal and leadership skills, with the ability to motivate and empower team members

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
