    Make an Impact as a Family Navigator

    The Family Navigator position will work with families being served through CCG programs and the Help Me Grow North Texas program. Working with families applying for services within targeted counties, the Family Navigator will connect families in an effective and timely manner to needed resources, education, and referrals to support services. They will connect with prenatal and new parents for education, assessment of risk factors and refer to resources and support agencies to ensure the goals are met in a timely manner. The Family Navigator is responsible for generating accurate and timely reports. Will report to the Family Navigation Manager with a high demand for independent judgment.

    Responsibilities and Essential Job Functions

    • Process, manage and close referrals within the required time frames
    • Determine the families' level of risk
    • Identify and provide community referrals based on the needs of the family
    • Distribute appropriate parent education material, including information on quality child care and the importance of early development. Provide information on programs and services such as the Positive Parenting Program (Triple P), Period of Purple Crying, postpartum depression, child development, home visiting, etc.
    • Must adhere to all policies and procedures, this includes CCG and all funding sources
    • Responsible for identifying and recruiting organizations that will partner with CCG to provide the program to their clients in individual or group settings
    • Work closely with families on the CCG waiting lists to connect with Pre-K or other community-based programs they are eligible for
    • Educate parents on the process of enrolling in child care
    • Work with CCG's Inclusion Specialists to connect families to ECI or other services
    • Attend training programs to learn about other partners as available and appropriate.
    • Responsible for ongoing professional development to keep abreast of current practices.
    • Responsible for assessing families using social determinants of health, the Ages and Stages Questionnaire (ASQ), and other assessment tools as needed.
    • Provide referrals to a variety of social services agencies and clinical programs, as appropriate.
    • Coordinate and attend group information and training sessions.
    • Ensure all screenings are conducted in a timely and confidential manner, according to the specifications of the grant.
    • Network with all social services agencies in the catchment area to attain knowledge of available services to refer families to all appropriate agencies.
    • Maintain current agency information in a database.
    • Refer families to all appropriate partner agencies and other social service agencies and ensure follow up.
    • Responsible for working individually and/or as a member of a team to meet the requirements of the funder.
    • Responsible for generating accurate and timely reports for the funder, including, but not limited to, individual and group training information, risk assessment reports, referrals, progress reports, vignettes, success stories and any other requested information.
    • Prepare materials for special reports, newsletters, and parent or provider educational materials as requested.
    • Represent the agency at workshops, conferences, and other events, and participate in staff development activities as assigned.
    • Keep abreast of current child development issues and share information and resources with parents.
    • Immediately notify the Family Navigation Manager of any concerns or complaints by clients, providers, and others.
    • Performs other duties as assigned.

    Education and Experience

    • AA or Bachelor's Degree in Child Development, Early Childhood, Parent Education, Social Work/Services or closely related field or equivalent, required.
    • Five years' work experience in child care, social work or parent education programs, with experience in new program implementation preferred.
    • Self-starter with demonstrated ability to work independently as well as in the framework of a team.
    • Good working knowledge of social service agencies and resources available throughout North Texas.
    • Excellent telephone and communication skills, competence in data processing, word processing and spreadsheet applications required.
    • Must be able to work well under pressure and meet deadlines as required.
    • Bilingual in English and Spanish preferred.

    Travel and Compliance

    • Must have a valid driver's license if applicable
    • Driving record must comply with CCG's automobile insurance carrier requirements
    • Must have liability insurance coverage and reliable personal transportation
    • Ability to travel locally or out of town as required
    • Mileage reimbursement is offered when applicable
    • Must submit to and successfully pass all required CCG and Partner Program background checks throughout employment. These include FBI, Fingerprinting, Neglect and Abuse, Federal, State, Local, and Sex Offender background checks. Background check requirements may be expanded at the discretion of CCG and Partner Programs
